## Summary

**Extends temporary telehealth recertification rules for Medicare hospice through December 31, 2027.** It would also require hospice recertification claims done by telehealth to include a telehealth modifier or code starting January 1, 2026. - Families and hospice patients: Would keep telehealth as an option for hospice eligibility checks and recertifications through Dec. 31, 2027, so remote recertifications remain possible. - Hospice providers and clinicians: Telehealth recertifications on or after Jan. 1, 2026 would be limited in cases where the area has a Medicare hospice enrollment moratorium of at least 6 months, the hospice is under enhanced oversight, or the clinician is not enrolled in Medicare enrollment rules and has not opted out. - Medicare billing and program administration: Hospice claims for telehealth recertifications would need one or more Secretary-specified modifiers or codes starting Jan. 1, 2026 to denote the telehealth modality.
