## Summary

**This bill would create a new federal model to expand access to specialty health care through digital modalities** for Medicare, Medicaid, and Children's Health Insurance Program enrollees in rural and underserved areas. It would test agreements with large nonprofit provider networks to deliver specialty services remotely while coordinating care with patients' primary care providers. - Rural and underserved beneficiaries: Medicare, Medicaid, and CHIP enrollees located in designated rural or underserved areas would be able to receive specialty care via telehealth and other remote technologies, coordinated with their primary care provider. - Provider networks: Selected networks must be nonprofit 501(c)(3) entities that include at least 50 Federally Qualified Health Centers, rural health clinics, critical access hospitals, or rural emergency hospitals, with at least half in rural areas. Networks must have multi-region experience and the ability to collect and share data for evaluation. - Primary care providers: Local primary care clinicians would coordinate remote specialty services, increasing integration of specialty care into community-based care.
