## Summary

**Sets a minimum payment floor for Medicare remote patient monitoring (RPM)**. It also requires real-time clinician availability, electronic health record compatible data transmission, and a congressionally mandated evaluation of RPM costs and savings. - Rural providers: Guarantees practice expense and malpractice geographic indices of at least 1.00 for RPM payments for services furnished on or after January 1, 2026. The floor is applied without a budget-neutral offset. - Medicare beneficiaries: RPM payment will be conditioned on a physician or qualified professional being available in real time to respond to physiologic anomalies. RPM data must be transmitted in a format compatible with electronic health records when needed. - Medicare oversight and policymakers: Requires collection and reporting of RPM data and a congressional report due within five years covering a four-year period beginning January 1, 2026. The report must estimate Medicare savings from earlier interventions and improved medication adherence and detail practice expenses for cellular connectivity and platform maintenance. *Expected to increase Medicare spending for RPM because the geographic-payment floor is not applied in a budget-neutral manner.*
