## Summary

**A grant program to treat diabetes in medically underserved urban and rural communities.** This bill would create funding for providers to deliver routine diabetes care, manage complications like kidney, eye, and foot disease, and run culturally and linguistically appropriate education and outreach. - Families and patients in medically underserved areas would gain expanded access to routine diabetes services, education, eye and foot care, and kidney disease treatment. - Community-based organizations, public or nonprofit health providers, Federally Qualified Health Centers (FQHCs), rural health clinics, and State, local, or Tribal health departments would be eligible to apply for grants. - Grantees must offer services in languages and cultural formats suited to the population and conduct outreach so communities know about available services. - The Secretary must aim for an equitable geographic distribution and balance urban and rural needs. The program would be funded by annual appropriations for FY2026 through FY2031.
