## Summary

**Creates a clear, authorized way for family caregivers to access a Medicare beneficiary's personal health information by phone.** This bill would require Medicare to run outreach, use a designated CMS authorization form, and set fraud-protection practices so caregivers can get PHI through 1-800-MEDICARE when a beneficiary gives permission. - Family caregivers would be able to call 1-800-MEDICARE to obtain a beneficiary's personal health information after the beneficiary completes the CMS-10106 authorization form. Medicare must train phone operators and collect caregiver feedback on the process. - Medicare beneficiaries would receive outreach and easy-to-understand notices about the option on Medicare.gov, in plan-specific materials, and in other prominent formats. The Department of Health and Human Services Office of Inspector General must publish best practices to prevent fraud and speed investigations within one year. - Plans, providers, and state support programs would see the authorization form and option included in notices and outreach, and the Secretary must, where practicable, coordinate with State Health Insurance Assistance Programs and the Administration for Community Living.
