## Summary

**creates a federal framework to improve cancer survivorship outcomes.** It would set standards, new payments, coverage rules, grants, and employment supports to help people live with and after cancer. - Cancer survivors and families would receive standardized survivorship care plans, treatment summaries, survivorship navigation, and psychosocial supports. Medicare would add a paid benefit for cancer care planning and coordination, initially paid at the same rate as transitional care management services. - Children and adolescent survivors would get Medicaid transition services with at least two survivorship transition visits per year. Medicaid and CHIP would be required to cover cancer fertility services with no cost-sharing and long-term storage for preserved material of at least 15 years for minors and 10 years for adults; the fertility coverage is effective October 1, 2026. - Workers and caregivers would benefit from a Department of Labor grant program that funds nonprofits to provide employment supports like transportation, childcare, paid-leave assistance, help applying for Social Security, and referrals to workforce training. - Providers and payers would face a new alternative payment model requirement for Medicare and Medicaid to be developed within one year, with design details reported to Congress and explicit inclusion of genetic counselors and mental health professionals. - Community groups, NCI centers, and state programs could get grants starting within 18 months to expand quality-of-life services and navigation. The bill also creates an Office of Cancer Survivorship at the National Cancer Institute and requires a Government Accountability Office study to begin within 6 months.
