## Summary

**Alternative payment models for perinatal care**. This bill would create a Perinatal Care Alternative Payment Model Demonstration Project run by CMS to let states test Medicaid and CHIP payment and coverage changes from fiscal year 2027 through 2031 to improve maternal outcomes. - Pregnant and postpartum people: Care would be designed to cover the entire pregnancy and the one-year postpartum period and encourage teams that include midwives, doulas, lactation consultants, and perinatal health workers. - States: States could apply to test risk-stratified payment models and coverage changes under Medicaid and the State Children's Health Insurance Program during 2027–2031. - Providers and settings: The project promotes inclusion of nonhospital birth settings, freestanding birth centers, federally qualified health centers, and rural health clinics, and allows reimbursement of doulas under state plans or waivers. - Data, equity, and evaluation: CMS would evaluate maternal mortality, severe maternal morbidity, spending, and patient experience with data broken down by race, ethnicity, language, income, geography, and insurance type and must report findings and recommendations to Congress after the project ends.
