## Summary

**Creates Medicare coverage for preventive home visits.** This bill would let Medicare pay for a professional visit to a beneficiary's home to assess hazards, identify health risks, and make referrals to improve mobility, balance, strength, or nutrition. - Seniors and Medicare enrollees: Would be eligible for a home visit delivered in-person, remotely, or both that assesses the home, identifies risk factors, and provides referrals for interventions or home modifications. - Providers and care teams: The Secretary would define who counts as a qualified professional or team. Medicare would pay a bundled rate for the visit and any connected referrals, equal to 100% of the lesser of the actual charge or the amount the Secretary sets. - Program rules and timing: Visits provided more often than allowed would not be covered. The coverage would apply to services furnished on or after January 1, 2027.
