## Summary

**Zero-percent loans to rebuild and renovate small rural hospitals** would create a new Rural Hospital Revitalization loan program in the community facilities direct loan authority to fund replacement, improvement, or renovation of eligible rural hospital campuses. The program ties eligibility to location, licensing history, and basic financial strength and pairs loans with targeted technical assistance to help hospitals stay open and improve access to care. - Hospitals: Eligible hospitals must be on a campus in a county under 20,000 people and at least 35 miles from the nearest hospital or 15 miles in mountainous or secondary-road areas. They must be a critical access hospital, a rural emergency hospital, or licensed continuously for at least 30 years, and show at least 30 days cash on hand and a debt service coverage ratio of 1.2. - Rural communities and patients: Loans aim to preserve or expand primary care and emergency services and prioritize places with very low population density or where half or more of recent patient care was Medicare, Medicaid, or self-pay. - Loan terms and support: Loans carry 0 percent interest for the first five years with repayment amortized over up to a 40-year term. After five years the Secretary assesses refinancing into the standard program at prevailing rates or may allow one-time renewals and extra relief when rates exceed 2.5 percent. Technical assistance grants from HRSA and USDA programs are available to support operations and financial stability during the zero-percent period.
