## Summary

**This bill would create a new Medicare benefit for cancer care planning and coordination services** that funds individualized survivorship care plans and pushes federal programs to expand supports for cancer survivors. It focuses on coverage, community grants, research, and payment and IT guidance to ease transitions from treatment to long-term survivorship care. - Would let Medicare cover development and delivery of individualized treatment and survivorship care plans at diagnosis, treatment changes, end of active treatment, and recurrence. Payment parity with transitional care management would be set equal to the Medicare rate for CPT code 99496. - Would require Medicaid and CHIP to cover healthcare transitions for childhood and adolescent cancer survivors, including at least two survivorship transition visits per year. It would also require coverage of cancer fertility services and ban cost-sharing in Medicaid and CHIP, with CHIP fertility coverage effective October 1, 2026. - Would create a Comprehensive Cancer Survivorship Program with grants for state cancer programs, NCI-designated centers, and community groups to fund navigation, mental health, nutrition, physical activity, and screening supports. It would establish an Office of Cancer Survivorship at the National Cancer Institute, require a GAO study within 6 months, and start the quality-of-life grant program within 18 months.
