## Summary

**This bill would create an experimental CMMI model that uses a _bundled payment to fund food-is-medicine services_ aimed at reducing diet-related chronic disease.** It would test integrated nutrition, care coordination, remote monitoring, and healthy-food provision for Medicare, Medicaid, and CHIP enrollees during a multi-year pilot. - Eligible individuals would receive a one-year package of services including personal risk assessment, nutrition counseling, care coordination, telehealth and remote monitoring, lifestyle programs, and provision of healthy, nutrient-dense foods, all without deductibles or copays. Participants who do not engage can be disenrolled. - The model would select at least five eligible programs for a minimum of two years each and pay a single bundled payment to cover APIM services. Beginning in the model's third year, selected programs may be required to bear financial risk for performance. - Programs must collect and submit health and utilization data, including quarterly weight, blood pressure, and blood glucose, for evaluation of cost savings and effectiveness. Purchasing priority goes to fresh or minimally processed plant foods and prefers locally produced or regenerative-agriculture produce, and the model must run for at least five years.
