Medical Form Required for USDA Hires
If you apply for Marketing and Regulatory Programs commodity grading or inspection jobs, USDA's APHIS will require you to provide medical information using MRP Form 5 (Self-Certification Medical Statement). The agency says this collection covers 175 respondents with a total burden of 31 hours and is used to determine physical fitness for positions that have medical or physical fitness requirements under 5 CFR part 339.
