Applying for Social Security benefits

To apply for retirement, survivors, or disability benefits you must complete SSA application tools (paper forms SSA-1, SSA-2, SSA-16; MCS interviews; or the internet iClaim). SSA estimates 6,167,621 total respondents annually for these application modalities, a combined total burden of 1,443,157 hours, and a theoretical opportunity cost of $73,805,710; SSA requests comments on these collections by April 10, 2025.

Overpayment waiver and repayment forms

If you were overpaid Social Security or SSI, you can use Form SSA-632-BK to request a waiver of repayment or Form SSA-634 to request a change in your monthly overpayment recovery rate. SSA estimates 400,000 respondents for SSA-632-BK (60 minutes each) and 100,000 respondents for SSA-634 (45 minutes each) and asks for comments on these collections by April 10, 2025.

Right to appeal disability denials

If SSA denies your disability claim, you can request a hearing by filing Form HA-501, using the Modernized Claims System (MCS) or SSI Claims System, or by using the internet i501 application. SSA estimates a total of 444,723 respondents for these hearing requests (162,904 for HA-501/MCS and 281,819 for i501) with an estimated combined burden of 97,606 hours and requests comments by April 10, 2025.

Requesting reconsideration of denials

If you disagree with an initial determination on Social Security, SSI, Special Veterans Benefits, or Medicare matters, you can request reconsideration using Form SSA-561-U2, MCS/SSI Claims System, or the internet i561. SSA estimates 786,356 respondents annually for reconsideration (157,014 total burden hours) and reports a theoretical opportunity cost of $9,036,240; SSA requests comments by April 10, 2025.

SSI payments accuracy reviews

If you receive Supplemental Security Income (SSI) and are randomly selected, SSA will contact you for a phone interview using Form SSA-8508-BK (e8508) to verify non-medical eligibility factors and payment amounts. SSA estimates each selected interview takes 60 minutes, reports 4,300 total annual respondents for this review, and requests public comments on the collection by May 12, 2025.
