If you are an adult victim of human trafficking, the Administration for Children and Families is extending the Request for Certification of Adult Victims of Human Trafficking form (OMB #0970-0454) for 3 more years so you can continue to get certification-based support without interruption through April 30, 2028. The extension continues use of the form that previously expired April 30, 2025 and involves no new costs.
