DEA Registration Revoked for Physician

DEA Certificate of Registration No. FK8011063 issued to Mohan Kaza, M.D., is revoked, removing his DEA registration and associated authority to prescribe or dispense controlled substances under that registration. The revocation is ordered by the DEA and takes effect June 6, 2025.

DEA denied any pending applications of Mohan Kaza, M.D., to renew or modify his registration and denied any other pending applications for additional registration in Michigan. That denial is part of the same Order effective June 6, 2025.

Order Stops Unsafe Prescribing Practices

The DEA found that between January 17, 2024 and April 17, 2024 the physician issued seven controlled-substance prescriptions to two undercover individuals without proper assessments, coached them to provide false medical histories, dispensed at least one tablet without a prescription, and charged higher cash fees for higher dosages. The Order revokes the registration to protect public safety and stop those unsafe prescribing practices immediately.
