If you apply to serve on a USDA advisory committee or a Research & Promotion (R&P) board, you must complete Form AD-755 to report your principal place of residence, employer, and major sources of income. The Department estimates 6,500 respondents annually with a total burden of 3,250 hours; public comments on this collection are open through September 8, 2025.

AD-755 used for background clearances

The information provided on AD-755 will be used to conduct background clearances for all candidates for appointment to USDA advisory committees and boards, including checks for delinquencies to USDA and House and Senate lobbyist checks. The clearance is required for candidates applying for appointment by the Secretary of Agriculture.

Optional demographic questions for outreach

Form AD-755 includes optional Race, Ethnicity and Gender (REG) questions plus veteran and disability data to help the USDA and the Office of Assistant Secretary for Civil Rights build demographically balanced advisory committees and guide outreach.
