High-Income Medicare Surcharges (IRMAA)

If your modified adjusted gross income is above $109,000 (individual filer) or $218,000 (joint filers), you may pay an income-related monthly adjustment amount (IRMAA) on top of the standard premium. Depending on income, beneficiaries can be responsible for about 35%, 50%, 65%, 80%, or 85% of Part B costs, with total monthly premiums shown in the notice (for example, up to $689.90 per month for the highest bracket in 2026).

Immunosuppressive Drug Coverage Premium

Beginning January 1, 2026, people who are 36 months post-kidney-transplant and elect Part B coverage only for immunosuppressive drugs pay a monthly premium of $121.60 (the amount can be higher if IRMAA applies). Electing this coverage lets you keep drug coverage even after full Medicare eligibility ends.

Medicare Part B Premium Rises

Starting January 1, 2026, the standard Medicare Part B monthly premium is $202.90. This is $17.90 (9.7%) higher than the 2025 standard premium of $185.00.

Part B Annual Deductible Set

Beginning January 1, 2026, the Medicare Part B annual deductible is $283.00 for all Part B beneficiaries. You must meet this deductible before certain Part B benefits begin to pay.

Hold-Harmless Protection for Social Security Recipients

For beneficiaries whose Part B premium is deducted from Social Security payments, premium increases may be reduced so the individual's net Social Security payment does not drop. The hold-harmless rule does not apply to beneficiaries who pay an income-related monthly adjustment amount (IRMAA).

Final $0.20 Repayment Added to Premium

The 2026 Part B premium includes a $0.20 monthly repayment amount required under law; this is the final repayment amount to repay prior transfers. This $0.20 is added to the otherwise determined premium each month.
