TRICARE Provider Application Burden
If your corporation or foundation wants to become a TRICARE/CHAMPUS Corporate Service Provider, you must complete form DD 3030 (OMB Control Number 0720-0020). The DoD estimates about 6,000 businesses will each spend about 20 minutes (totaling 2,000 annual burden hours) to apply; the collection is voluntary and the public comment period ends December 26, 2025.
