Form to Request Review of Disability Annuity
If you retired under disability annuity rules (or are a survivor or beneficiary), you can use OPM form RI 20-123 to ask the Office of Personnel Management to review the computation of your disability annuity. The form applies to retirees who served as law enforcement officers, firefighters, nuclear materials couriers, air traffic controllers, Congressional employees, Members of Congress, Capitol and Supreme Court police, or customs and border protection officers; OPM will review the computation and, if you are entitled, process any increased benefit or amounts accrued but unpaid.
