Published Date: 12/8/2025

Notice

Summary

The Centers for Medicare & Medicaid Services (CMS) wants your thoughts on their plan to collect some info from the public. This is a routine check to make sure the questions they ask are useful and not too much work. If you have ideas or concerns, you’ve got until January 7, 2026, to speak up—no cost to comment, just your time!

Hospitals must give MOON notice

If you have Medicare and you receive more than 24 hours of hospital observation services, the hospital must give you a standardized written notice called the Medicare Outpatient Observation Notice (MOON) telling you your stay is outpatient and explaining the implications. CMS lists this as Form CMS-10611 (OMB control number 0938-1308), used annually with 2,073,991 responses and a total annual burden of 518,498 hours.

HOS field test may change Star Ratings

CMS will run a Medicare Health Outcomes Survey (HOS) field test (Form CMS-10861, OMB control number 0938-1464) to collect data (50 respondents, 6,800 responses, total annual burden 1,700 hours). HOS measures feed into Medicare Part C Star Ratings (published each fall), and CMS will use the field test to inform possible changes to HOS content; any new measures would go through the Measures Under Consideration process and rulemaking before being added to Star Ratings.
