The Department of State wants to update the medical history and exam forms used by employees and their families working overseas. They’re asking for public feedback over the next 60 days before making changes official. This update won’t cost extra but will help keep health checks smooth and clear for about 2,000 people.

Forms decide overseas assignment eligibility
These forms collect medical history and exam results so a medical provider and the Department can make a medical clearance decision for initial appointment to an overseas assignment. The forms are also used to decide whether you or eligible family members will have appropriate medical and/or educational resources at the host mission.
You must complete medical clearance forms
You must complete forms DS-1843 and DS-1622 when applying for an overseas position (and again intermittently as needed). The Department estimates 2,039 respondents, an average of 1 hour per response, and that responding is required to obtain or retain the benefit of an overseas assignment.
New questions for non-foreign service employees
The updated DS-1843 and DS-1622 now include questions that collect employment agency information for non-foreign service agencies, which will apply to individuals from those agencies when they complete the forms.
