DD Form 93: Name beneficiaries and contacts

DD Form 93 (Record of Emergency Data) is used by Service members to designate beneficiaries for certain benefits if the Service member dies and to name who should be notified in an emergency or death. The form also guides disposition of pay and allowances if a member is captured, missing, or interned and designates who may direct disposition of the member's remains; this requirement is identified in DoDI 1300.18.

Time burden to complete DD Form 93

The Department of Defense estimates 1,739,012 respondents complete DD Form 93 once per year, with an average burden of 5 minutes per response and total annual burden hours of 144,918. The information collection is on occasion and comments on the paperwork burden are due by February 27, 2026.
