Active/Reserve Dental Info Collected
If you are an active duty or reserve member of the Armed Forces, your dental health status will be recorded on DD Form 2813 (Department of Defense Active Duty/Reserve Forces Dental Examination) and provided to your military organization. The form is used annually to track dental status as required by Department of Defense policy.
Civilian Dentists Must Complete DD Form 2813
Civilian dentists (and other civilian dental providers) will use DD Form 2813 to record their dental exam findings for service members and provide that information to the member's military organization.
