Medicare MLR Reporting & Sanctions
If you run a Medicare Advantage (MA) organization or a Part D Prescription Drug Plan (PDP), you must report the medical loss ratio (MLR) for each contract year using form CMS-10476. The statute requires an MLR of at least 85 percent, and plans that fail to meet that standard face sanctions including remittance of funds to CMS, a prohibition on enrolling new members, and possible contract termination; the collection is yearly, covers 660 respondents, and totals 40,356 annual hours. Comments on this information collection are due April 13, 2026.
