Military spouses must file DD Form 3145-4
If you are a military spouse seeking DoD jobs at GS-15 and below (or equivalent), you must complete the Military Spouse Priority Placement Program Self-Certification Checklist (DD Form 3145-4) to get priority consideration for competitive or excepted service positions at DoD activities in the U.S. and U.S. territories. You must provide evidence of appointment eligibility and proof of marriage to an active-duty service member (including a copy of permanent-change-of-station orders).
