TSGLI benefit application form

If you are a member or former member of the uniformed services who suffered a traumatic injury while in service, you use the SGLV 8600 form to apply for Servicemembers' Group Life Insurance–Traumatic Injury Protection (TSGLI) benefits. The form is authorized by 38 U.S.C. 1980A and 38 CFR 9.20 and is used to gather information so the uniformed services can approve or disapprove a claim; if approved, The Prudential Insurance Company of America pays the claim.

If your TSGLI claim was disapproved, you use the SGLV 8600a form to request an appeal of that decision. The form is authorized by 38 U.S.C. 1980A and 38 CFR 9.20 and is used by members or former members of the uniformed services to provide information for adjudicating TSGLI appeals.
