Up to $50B for Rural Health

A law signed July 4, 2025 authorized the Rural Health Transformation (RHT) Program with federal investment of up to $50 billion over five years to support as many as 50 State awardees. The program is intended to improve healthcare access, quality, and outcomes in rural communities.

State Funding Tied To Performance Reporting

State awardees will receive two funding streams: baseline funding distributed equally and performance-based workload funding allocated by scoring. CMS will re-calculate each State's technical score and workload funding amount each budget period based on the information States provide in required annual reports.

Quarterly and Annual Reporting Burden

Approved State awardees must submit quarterly and annual reports using Form CMS-10949 (OMB control number 0938-TBD). CMS estimates 50 respondents, 200 total annual responses, and 1,950 total annual hours of burden for this collection; comments on the proposed collection are due by May 11, 2026.
