Continued Harassment Reporting Duty
If your institution received NIH grant or cooperative agreement funds in FY2022 or later, the institution must notify NIH within 30 days when a principal investigator or key personnel is removed or disciplined for harassment. The notification must be submitted through the NIH Extramural Harassment Web Form and include the Authorized Organization Representative's name, the individual of concern, a description of concerns, actions taken, and any anticipated impact on NIH-funded award(s). OMB approval is being requested to extend the form for an additional 3 years (OMB #0925-0777; expiration listed as 2026-03-31).
Secure Web Form for Harassment Reports
Researchers, staff, and authorized representatives can use a secure, confidential NIH Extramural Harassment Web Form to report harassment related to NIH-funded research. There are no monetary costs to respond; each submission is estimated to take about 15 minutes, with total estimated annual burden of 60 hours (240 total annual responses: 140 private-sector grant recipients and 100 individuals).
