If you are a small business seeking SBTTAC services, you (or your field office) must be enrolled using the SBTTAC Intake Form (DOT F 4500). The form is collected once per client; the agency estimates 2,521 respondents and a total annual burden of 3,781.5 hours for respondents. The collected data is used to provide services like training, counseling, short-term loans and to help increase opportunities to participate in government contracts and subcontracts.
SBTTAC centers must submit monthly intake data
The SBTTAC Centers (6 centers) will fill and submit the intake information monthly to OSDBU; the agency estimates 72 responses (6 centers × 12 months) and an annual burden of 72 hours. Field offices will retain a copy of each intake form.
