Three-Year Extension of Worksheet Approval

USDA APHIS is requesting OMB approval to continue using the Spongy Moth Identification Worksheet and Checklist for an additional 3 years under OMB Control Number 0579-0104. APHIS estimates an average burden of 0.362 hours per response, about 2,500,100 respondents, 3 responses per respondent (7,500,250 responses total), and an estimated total annual burden of 2,711,543 hours. Comments are requested by June 22, 2026.

Self-Inspection Checklist Requirement for Moves

If you move outdoor household articles from a spongy moth quarantined area to a non-quarantined area, you may use and must be prepared to present a signed self-inspection checklist that documents the inspection and is kept in the vehicle during the move; APHIS recommends keeping a copy of the signed checklist for at least 5 years. The specimen-for-determination worksheet must accompany suspicious moths sent for DNA analysis to APHIS laboratories.
