Who Must Sign Form HHS-690

If you apply for or receive HHS financial assistance, you must provide an Assurance of Compliance using Form HHS-690 (OMB Control Number 0945-0008). The collection is being revised before the current approval expires on April 30, 2026, and respondents provide the form when they apply for or receive new HHS funds.
