More Medicare discharge info on home care
Hospitals would need to give more discharge information to Medicare patients. For discharges on or after January 1, 2026, they would list home health services. If staff think a patient will need home health, they would share local home health agency options. If staff think a patient may qualify for hospice, they would share Medicare hospice programs that serve the patient’s area. This change is informational and would not change Medicare benefit amounts.
