- Families and minors: Families with transgender children would be unable to use federal funds to obtain or be referred for the listed surgeries, implants, puberty blockers, or cross‑sex hormones for people under 18 when federal money is involved.
- Health care providers: Providers could not receive federal reimbursement or referrals for performing the specified procedures on patients under 18.
- Federal programs and payers: Federal programs and grants would be barred from paying for or referring these procedures for minors.
- Covered procedures and exceptions: "Specified gender transition procedure" includes a long list of surgeries, chest and gluteal implants, puberty‑blocking drugs, and supraphysiologic doses of testosterone or estrogen. The bill exempts treatment for diagnosed precocious puberty and certain genetically or medically verifiable sex development disorders, chromosome or hormone abnormalities, or injuries and life‑threatening physical conditions determined by a physician, and it excludes procedures done only to alleviate mental distress.

Ban on federal funding for minors' transitions
If enacted, this bill would bar federal funds from being used to provide, refer for, or reimburse providers for "specified gender transition procedures" for people under 18. The covered list would include many surgeries (for example, mastectomy, phalloplasty, vaginoplasty, orchiectomy, hysterectomy), chest and gluteal implants, puberty blockers, and supraphysiologic doses of testosterone or estrogen used to change the body. The rule would operate notwithstanding any other law. Narrow exceptions would allow puberty blockers only to treat precocious (early) puberty and allow medically necessary care for verified disorders of sex development or to prevent imminent death or major bodily harm. Procedures done only to ease mental distress would not be excepted. If passed, parents and guardians of minors seeking these services could face higher out-of-pocket costs or reduced access, because federal programs could not pay, refer, or reimburse for these services.
