Makes Medicare telehealth for cardiopulmonary rehabilitation permanent and expands access to patients' homes. The bill would codify COVID-era in-home telehealth flexibilities for cardiac and pulmonary rehabilitation, remove certain geographic limits for specified originating sites, and require the HHS Secretary to set standards for designating homes as provider-based hospital outpatient locations.

- Medicare beneficiaries: Would preserve access to home-based cardiopulmonary rehabilitation, pulmonary rehabilitation, and intensive cardiac rehabilitation delivered via two-way audio-visual telehealth.
- Hospitals and providers: Would allow a patient's home to be designated as a provider-based location of a hospital outpatient department and ensure these rehab programs are covered telehealth services under Medicare.
- HHS and regulators: Directs the Secretary to issue rulemaking defining standards for home provider-based designation under Hospital Without Walls waivers and to include these programs under applicable telehealth authorities. The standards would take effect on the date they are issued.

If enacted, Medicare would let you get cardiac or pulmonary rehab at home by two-way video. Your home could count as a hospital outpatient location for these services. A doctor, physician assistant, nurse practitioner, or clinical nurse specialist could join by video. Medicare would also pay hospitals for telehealth video services. HHS would have to issue rules on how a home becomes a provider-based hospital location.
