- Families and Medicare beneficiaries: Could gain covered access to tests meant to detect pre-symptomatic and early-stage Alzheimer’s disease and related dementias, with coverage tied to tests furnished on or after Jan 1, 2028.
- Test developers and labs: Would need FDA clearance or approval under pathways such as 510(k), 513(f)(2), or 515 to qualify. The bill explicitly allows a wide set of test types, including genomic sequencing blood tests, single-analyte tests, cell-free nucleic acids, multiplex panels, whole genome or exome sequencing, protein expression assays, and medical imaging based on biospecimens.
- Medicare program and providers: Adds a new covered category to the Social Security Act and updates Medicare payment rules so qualifying ADRD early detection screening tests are eligible for Medicare payment.

Medicare coverage for early dementia tests
If enacted, Medicare would cover certain early Alzheimer's and dementia screening tests starting on or after January 1, 2028. Tests would qualify only if cleared under FDA 510(k), classified under 513(f)(2), or approved under 515. Covered tests would include genomic sequencing blood or blood-product tests and other equivalent tests based on blood, tissue, urine, or other biospecimens. The bill would add these tests to Medicare payment rules, which could lower out-of-pocket costs for beneficiaries who receive them.
