The Secretary must do a full review of VA health care at least every 4 years. Each review must check how many veterans want care by geographic area, what VA hospitals and clinics can provide, what community and contract providers can do (how many, where, and what services), what other federal systems and non‑contract providers can offer, how VA’s academic partners help, how access and quality rules affect capacity, and how many appointments happen at VA vs non‑VA providers. The Secretary must send these reviews to the Veterans’ Affairs and Appropriations Committees in both the Senate and the House. The reviews must be used to judge the strength of VA’s provider networks, to shape the VA budget, and to suggest changes to access and quality rules. The VA budget submitted each year must reflect the latest review and actual use of care inside and outside the VA. Not later than 1 year after the Caring for Our Veterans Act of 2018 became law, and at least every 4 years after that, the Secretary must send Congress a 4‑year strategic plan. The plan must forecast demand by area, the capacity at each VA medical center, and community care capacity. Preparing the plan must include checking access and quality rules, using the market reviews, looking at special or growing needs, talking with other agencies and stakeholders, spotting trends, setting short- and long‑term priorities, considering a veteran satisfaction survey, reviewing programs and future demand, and reviewing how closed or fixed service lines are being handled with outside care. The Secretary must also lead VA’s reorganization to build a high‑performing integrated health network, plan and buy facilities or leases, and create a multi‑year budget process that forecasts future costs.
