The Director of the Institute must work with the Institute’s advisory council to set up, update, and run centers for arthritis and musculoskeletal diseases. The work includes paying for staff and any patient care needed for research. "Modernization" means changing, fixing, expanding, or equipping buildings so they can be used as these centers. Each center must be based at one institution or a group of institutions and meet rules set by the Secretary. Centers must do both basic and clinical research (including implants and orthopedic methods), train doctors and other health workers, offer continuing education for clinicians, and give public information about early detection, treatment, and avoiding unproven or unsafe products and methods. Centers may also test better ways to detect and refer people at risk, share research results and standardize patient records, and help communities refer patients for care. The Director should spread centers fairly across the country and consider centers for children. Support can last up to 5 years and may be renewed in additional 5-year periods after peer review. By October 1, 1993, the Director had to create a center focused on children’s arthritis and musculoskeletal research and care.
