To get a grant or contract, an eligible group must send an application to the Secretary in the form and at the time the Secretary asks for. The application must say how the group will use the money and show that the plan fits with relevant Federal, State, or regional health professions plans. The application also must list clear outcome measures the project will be judged by, focused on the health workforce needs it will meet, and the grantee must meet those measures. It must describe connections with schools and health care programs, including training for other health workers when needed. When possible, grantees should link with providers who care for underserved communities.
