The federal government cannot give the yearly grant money unless the state agrees to limits on how it will be spent. The state must promise not to use the money for inpatient hospital care, to give cash directly to people, to buy or make major changes to land or buildings (small remodeling is allowed), to buy major medical equipment, or to meet a rule that requires the state to spend its own money to get federal funds. The state must also promise to spend no more than 5% of the grant on administrative costs for running the program.
