If you operate a Comprehensive Outpatient Rehabilitation Facility (CORF), an outpatient physical therapy/speech-language pathology (OPT/SLP) provider, a Rural Health Clinic (RHC), or a Portable X‑Ray (PXR) supplier, CMS plans to let you submit a self-attestation for recertification instead of undergoing a State Survey Agency (SA) onsite recertification survey every 6 years. You would complete a dedicated attestation form before your recertification due date, and a properly completed and timely submitted form would be accepted by the applicable State Survey Agency.
