Physician Certification Required for SNF Payment

If you have Medicare Part A and need post-hospital skilled nursing facility (SNF) care, a physician or other authorized practitioner must certify and periodically recertify that you require an SNF level of care. That certification and recertification is a condition for Medicare Part A payment for posthospital SNF services (described under section 1814(a) of the Social Security Act).

Paperwork Burden on Providers for SNF Certifications

CMS is seeking OMB approval to continue collecting physician certifications/recertifications (Form CMS-R-5, OMB control number 0938-0454). The notice reports 3,882,413 respondents/responses and a total annual burden of 480,957 hours for this collection.
