Researchers Must Complete RDC Security Forms

If you are a researcher who needs access to confidential NCHS data, you must complete data security forms. For access at a Research Data Center (RDC) complete the Data Access Form and the Designated Agent Form; for access through the NCHS Virtual Data Enclave (VDE) complete the VDE Data Use Agreement Form, the Data Access Form, and the Designated Agent Form. CDC estimates a total annual burden of 110 hours (1 hour per response for 110 responses) and 330 hours over the three-year approval period; there is no cost except your time and a possible nominal notary fee.
