These new listings are intended to address very serious conditions that can progress quickly and limit an adult's ability to do any gainful activity or cause marked and severe limitations in a child's function.

The rule maintains a threshold recognizing an ejection fraction (EF) below 30 percent as severely abnormal and keeps the listing 4.02C criterion of an EF of 20 percent or less for identifying listing-level chronic heart failure.

When the Rule Applies

This rule takes effect on October 30, 2026. The Social Security Administration will use the new cardiovascular listings for any applications filed on or after October 30, 2026 and for claims or continuing disability reviews that are pending on or after that date; it will also apply to cases remanded by a court after that date.

Sex-Specific LVEDD Cutoffs

For chronic heart failure under listing 4.02A1, the left ventricular end diastolic dimension (LVEDD) threshold during a period of stability is now greater than 6.8 cm for men and greater than 6.1 cm for women.

Cardiac MRI Accepted as Evidence

The Social Security Administration added cardiac magnetic resonance imaging (cardiac MRI) to the list of appropriate medically acceptable imaging tests to evaluate chronic heart failure.

Clarify Implanted Mechanical Support

Listings 4.02D1 and 104.02D now specify an implanted mechanical circulatory support device (MCSD) for the listing; devices using Impella technology that are intended for short-term use do not meet the listing requirement because they are not implanted for long-term support.

iFR Added for Ischemic Testing

The rule adds instantaneous wave-free ratio (iFR) as an accepted physiological measure (new listing 4.04D2) alongside fractional flow reserve (FFR) to assess coronary stenosis severity for ischemic heart disease evaluations.
