The Department of the Treasury will submit the following information collection requests to the Office of Management and Budget (OMB) for review and clearance in accordance with the Paperwork Reduction Act of 1995, on or after the date of publication of this notice. The public is invited to submit comments on these requests.

Form to claim decedent's Treasury securities

If you are claiming Treasury securities or payments after someone dies and the estate was administered or is being settled under State law without a court-appointed representative, you may need to complete FS Form 5394. The Treasury estimates 18,500 respondents, each taking about 30 minutes, for a total annual burden of 9,250 hours.

Refund form for chain-letter savings bonds

If you bought U.S. Savings Bonds or Notes through a chain letter scheme, you may use FS Form 2966 to request a refund of the purchase price. The Treasury estimates 240 respondents will use this form, it takes about 8 minutes to complete each time, and the total annual burden is 32 hours.
