Required Medical Clearance Update Form
If you are Chief of Mission personnel or an eligible family member, you must complete the DS-3057 Medical Clearance Update Form to maintain medical clearance. Online MyMED Portal and Security
The DS-3057 will be completed digitally through the myMED portal (MED Provider Portal). Access requires identification verification and two-factor authentication; adults 18 and older must use their own account, and respondents will complete entries for any children ages 17 and younger.
