Updated VA Scholarship Forms Required
If you are applying to or already in the VA Health Professional Scholarship Program (HPSP) or the Visual Impairment and Orientation and Mobility Professionals Scholarship Program (VIOMPSP), you must submit updated VA Forms (VA Forms 10-0491, 10-0491c-e, 10-0491g-k) to prove eligibility and track service. These forms are submitted once annually as part of the program application and verification process.
Paperwork Time Burden Increase
The VA estimates a total of 2,050 annual responses and a total annual time burden of 8,017 hours for these scholarship program forms. For HPSP there are 2,000 responses with a total burden of 7,809 hours (about 3.905 hours per respondent); for VIOMPSP there are 50 responses with a total burden of 208 hours (about 4.15 hours per respondent).
Two Pilot Programs Removed
Two pilot scholarship programs and two associated forms have been removed from this information-collection package because the pilot programs ended since the last PRA clearance. The current collection reflects only the continuing HPSP and VIOMPSP forms.
