Summary

The Department of Labor, as part of its continuing effort to reduce paperwork and respondent burden, conducts a pre-clearance request for comment to provide the general public and Federal agencies with an opportunity to comment on proposed collections of information in accordance with the Paperwork Reduction Act of 1995. This request helps to ensure that: requested data can be provided in the desired format; reporting burden (time and financial resources) is minimized; collection instruments are clearly understood; and the impact of collection requirements on respondents can be properly assessed. Currently, the Office of Workers' Compensation Programs, Division of Federal Employees' Compensation, (OWCP/DFEC) is soliciting comments on the information collection for the CA-12, Claim for Continuance of Compensation.

Annual Verification to Keep Death Benefits

Form CA-12 is sent annually to beneficiaries receiving continuing death benefits to verify marital and/or beneficiary status so they remain entitled to benefits under the Federal Employees' Compensation Act (see 5 U.S.C. 8133 and 20 CFR 10.414). The notice confirms the CA-12 is used to monitor current marital status and potential for dual benefits.

CA-12 Paperwork Extended Without Change

The Office of Workers' Compensation Programs is proposing to extend, without change, the information collection for Form CA-12 (Claim for Continuance of Compensation). The collection lists 2,362 respondents, 2,362 annual responses, 197 total annual burden hours, and $1,603.00 in annual respondent cost (OMB Control No. 1240-0015). Comments on the extension must be received by October 19, 2026.
