Office of Personnel Management (OPM) offers the general public and other federal agencies the opportunity to comment on the reinstatement of an expired information collection request (ICR), Application for Death Benefits/Documentation and Elections in Support of Application for Death Benefits When Deceased Was an Employee at the Time of Death (CSRS).

If you are a survivor of a Civil Service Retirement System (CSRS) annuitant or federal employee, OPM requires submission of Standard Form (SF) 2800 to pay death benefits. OPM estimates 59,750 respondents will each spend about 45 minutes completing SF 2800, for a total estimated burden of 27,000 hours.

SF 2800A used for deaths in service; includes military elections

If the deceased was a federal employee who died while in service, OPM uses Standard Form (SF) 2800A to collect documentation and elections (including elections about military service) from survivors. OPM estimates 250 respondents will each spend about 45 minutes on SF 2800A, for a total estimated burden of 113 hours.
