Summary

This bill would require Medicaid to cover services from doulas, midwives, tribal midwives, and lactation support providers. It would let those services be delivered in homes, clinics, hospitals, birth centers, community settings, or by telehealth and would bar cost sharing for them.

  • Families: Pregnant and postpartum people would get Medicaid-funded prenatal, labor, postpartum, and breastfeeding support with no cost sharing.
  • Doulas: Doulas could serve as Medicaid providers if certified, keep certification with continuing education, and supply at least three former-client or two licensed-provider recommendations from the last five years.
  • Midwives and tribal midwives: Midwives qualify if authorized in a State plan or if they meet international midwifery education standards. Tribal midwives also qualify if authorized in a State plan or recognized by their tribe.
  • Lactation support providers: Providers must complete at least 20 hours of WHO/UNICEF-based foundational training or meet recognized categories on the U.S. Breastfeeding Committee chart to qualify.
  • States and timing: States would need to add these services to their Medicaid plans or waivers for care furnished on or after January 1, 2027, with extra time when new State legislation is required.
