(a) The Identification Bureau of the Division of Arkansas State Police shall maintain an index of the results of each applicant's criminal history check.
(b) The Department of Health shall develop forms that are approved by the bureau to be used for criminal history checks conducted under this subchapter.
(c) The department shall develop and maintain a database of determinations regarding applicants.
